Allied Wellness Collective is an integrated collaborative team of mental, behavioral, and other healthcare providers. Our team of clinicians and support members are dedicated to making sure our clients and patients receive the best possible care. To meet this goal. we have developed various policies and procedures to assist our providers and patients to understand their rights, responsibilities, and options available to them in specific situations.

Statement of Non-Discrimination

Allied Wellness Collective will not discriminate in the provision of health care services to an individual because the individual is unable to pay for the health care services; because payment for those services would be made under Medicare, Medicaid, or the Children’s Health Insurance Program (CHIP); or based upon the individual’s race, color, sex, age, national origin, disability, religion, gender identity or sexual orientation. Statement on Sliding Scale Policy

Allied Wellness Collective serves all patients regardless of ability to pay. Discounts for essential services are offered based on family size and income.
